If your child attends a New York City Public School, you will need their nine-digit student identification number (OSIS) to begin the survey online. You can find this number on your child's report card or student ID. If you're having trouble locating it, please feel free to reach out to your child's teacher for assistance.
Here's how to complete the survey:
Click on Take the 2026 NYC School Survey at
https://www.schools.nyc.gov/about-us/reports/school-quality/nyc-school-survey.
Enter a lowercase “f” followed by your child’s nine-digit OSIS number (for example: f123456789).
